Once you have decided to join the team, you will need to submit the following three items:

1. Mission Trip Agreement
2. Registration Form
3. $250 deposit per person.

Your spot on the team will not be reserved until all three items have been submitted. The Mission Trip Agreement and Registration Form can be completed by clicking the links above. The payment can be completed by using the payment portal below, or by placing a check in the normal Sunday offering and marking it as "Dominican Republic Missions".

Trip payments can be placed into the offering at Living Truth Church during Sunday morning services. Please put the memo "DR Mission Trip" on the check or giving envelope to ensure the payment is sorted correctly.
